in the process of creating a new skin i often come to a point where i want to add more and more things to a skin.
that's when i remind myself to an often quoted motto.

although not yet released CAD2.0 will be available for beta testing in february/march 2008.

I put the "limo" folder in my Winamp/Skins directory and it comes up in Winamp in the skins listing, but when I select it, I get the old, gray, boxy looking skin (same as Winamp Classic).

i thought it was all self-explanatory, but apparently it wasn't. as i've written this is a CD Art Display 2.0 skin. no winamp, no iTunes, no WMP no whatever. however CD Art Display is a frontend application that can be used together with all the players mentioned above (and quite a few more) to nicely show the cover art of the currently played song. i've linked to CD Art Display in the release notes in order to get some more info about CD Art Display.
now, if you take a close look at the screenshot you'll notice that there's a hotspot configured to show/hide all the additional elements (i.e. text, ratings and progressbar). so when your mouse-pointer enters the area at the top of the cover, it will change itself into a hand cursor. make a click there and you'll be shown all the missing elements. you can switch off the hotspot in the Skin Editor if you like.
the "clear box" as you call it, is the case for a back-cover or any picture CAD will find in the current song folder. you can assign certain search patterns in CAD's options to explicitly point to a folder.
please note, that this is a skin for CD Art Display 2.0. if you're running CD Art Display 1.0 not all features of this skin will be supported. as written in the release notes, check out the skin's forum topic to find the according and compatible skin version.
i hope there will be a day when CD Art Display skins will no longer be forced to share a gallery with winamp skins, but would get its own gallery instead. the days when CAD would work with winamp only were long gone and iTunes, WMP, foobar, qmp and so on users would finally be able to discover some cool skins for an amazing application. besides there would be no more questions on how to install a CD Art Display skin in winamp.

Put the "limo" folder in your C:\Program Files\CD Art Display\Skins folder.
Open CD Art Display, go to skins, choose "limo".
Play a song.
